Job Responsibilities:

• Assist with Healthier SG-related tasks at GP clinics.
• Collaborate with GPs for PCN and Healthier SG enrolments.
• Coordinate annual eye and foot screenings for diabetic patients.
• Provide administrative support for patient referrals to community services.
• Track and report on screening activities and patient adherence.
• Enter clinical data into Excel and support patients with HSG-related apps.
• Help GPs with patient care coordination with community partners.
• Work with PCN nurses to identify patients needing counseling.
• Offer basic lifestyle and behavioral advice to support patient care plans.
• Aid in survey administration and data analysis.
• Gather feedback on issues and manage patient transfers between clinics.
• Perform other duties as assigned by the Reporting Officer.

Job Requirements:

• Diploma in Healthcare related field
• Meticulous, friendly, and positive attitude
• Independent and a team player
• Able to travel and work on weekend at times

Other Information:

• Salary up to $3300 (depends on experience) + AWS + Bonus
• 5 days work week, Mon-Fri: 8am-4pm
